Asthma Education

Open to all North Hamilton clients. Participants will learn self-management strategies regarding their asthma. Breathing tests will be conducted.

 

Chronic Disease Self Management (Your Health In Your Hands)

Open to people living with long-term health effects/chronic disease, and their caregivers. Modeled on Stanford University's Chronic Disease Self Management program.

 

Diabetes Education

Open to persons with diabetes, with physician referral. Program is beneficial for diabetic education, including blood glucose monitoring, diet and physical exercise.

 

Diabetes Kitchen

Those who live with diabetes or who are at an increased risk of developing diabetes can come learn how to prepare healthier food.

 

Diabetes Physical Activity Group

If you live with diabetes or are at an increased risk of developing diabetes, our Activity Group will get you moving towards better health.

 

Maternal/Child Clinic

Open to women in transition, pregnant women, and mothers and their children (0-6 years) who don't have a family doctor and live in the North End.

 

Smoking Cessation

Open to anyone interested in learning more about quitting smoking. Offers support, access to information, and strategies and options for quitting.
