JOB TITLE: Foot Care Nurse Diabetes Program Niagara
RESPONSIBLE TO: Health Wellness Team Leader
SUMMARY OF FUNCTIONS:
The Diabetes Program Foot Care Nurse will provide preventative curative foot care to diabetes clients in the Niagara area. They will assist in the coordination of the day-to-day diabetes program foot care services at the Health Centre. The foot care nurse's approach to client care is evidenced-based and innovative and utilizes current therapeutic techniques within the determinants of health framework. This position works within an inter-professional team-based model in terms of both the clinical practice and program planning aspects of its responsibilities. A systems perspective and attention to continuous quality improvement are essential to the success of this position.
All foot care nurses will have a target of sixteen (16) clients seen (units of service) per full clinical day worked. The average appointment length will be 20 minutes with the exception of appointments where an interpreter is engaged or when examinations and procedures require more time. Time will be scheduled for non-direct client care as needed and activity will be encountered in the EMR.
MAJOR DUTIES:

Provide basic foot assessments and care for people living with diabetes in Niagara who are at low to moderate risk of foot complications relating to their disease.

Screen clients according to program criteria and ensure that clients referred to the program are triaged to the appropriate level of service.

Provide education around the care of the diabetic foot for people living with diabetes in Niagara region.

Assist in the provision of outreach foot care foot care at geographical accessible locations in Niagara.

To be a resource person to the staff, students and community regarding diabetic foot care.

To assist in the implementation and provision of diabetes foot care health promotion activities in the Community, including increasing the awareness of this service.

To provide clinical instruction to learners, nursing students and other health professions.

Screening for foot complications related to diabetes and referral to appropriate care providers as necessary.

To be a member of the multi-disciplinary team of health professionals at CHCs in Niagara providing family care to clients in the region.

Such other activities as may be assigned.
QUALIFICATIONS:

Current registration with the College of Nurses of Ontario.

Completed Basic and/or Advanced Programs in foot care.

Completion of Diabetic Program in Foot Care preferred.
-
College diploma and three years of foot care experience in a health care setting.

Possess excellent assessment, organizational and problem-solving skills.

Availability of transportation to outlying clinics

Excellent communication and interpersonal skills

POSITION TITLE: Chiropodist - Diabetes Program (Niagara)
RESPONSIBLE TO: Health Wellness Team Leader
SUMMARY OF FUNCTIONS:
To provide foot care services in the Health Centre and affiliated health care institutions, as well as to coordinate the day-to-day chiropody services at the Health Centre.
MAJOR DUTIES:
- Provide foot care assessment and treatment for clients living with diabetes at high risk of foot related complications.
- Provide community education around foot care to clients with diabetes.
- To be a resource person to the staff, students and community regarding foot care.
- To develop, implement and evaluate chiropody health promotion activities in the community, including increasing the awareness of this service.
- To provide clinical instruction to learners, and students of chiropody and other health professions as needed.
- To design and maintain a system to coordinate chiropody and orthotic activities provided at Community Health Centres in Niagara and at other affiliated health care institutions.
- To work with members of the multi-disciplinary teams in the Niagara area to provide comprehensive care including disease management and prevention for clients with diabetes.
- Other activities as may be assigned.
QUALIFICATIONS:
- Current registration with the College of Chiropodist of Ontario under the 1994 Chiropody Act.
- Post graduate preparation or equivalent experience of a minimum of 2 years practice.
- Demonstrated ability to be flexible and adaptable.
- Possess excellent assessment, organizational and problem-solving skills.
- Excellent communication and interpersonal skills.
